To reach Oaxaca's airport (OAX) you can hail any cab in the city or arrange
beforehand for a pickup.  They will come to your hotel or apartment early in the AM.  
They are reliable for an early pickup.
The hotels will arrange a pickup for you if you choose.  You can also contact a tourist
agency to book a cab to the airport or hire a private car through a tourist agency

Zocalo, corner of Hidalgo and Lopez

are vans serving as shared taxis  that provide an inexpensive ride into the city.
They will drop you wherever you want to go.

through a Oaxacan Tourist agency ahead of your trip. The driver will be at the
airport waiting for your arrival
You will receive a six months visa on the airliner, paid for through your ticket
price.  You will need picture ID, or birth certificate to enter Mexico.
You will need a passport to enter the United States.
